Not a healthcare communication channel. This website is a marketing and information site operated by Habit Layer LLC. It is not a clinical platform, patient portal, or HIPAA-covered entity. If you are a patient and you contact us through the web form on this site, that submission is a voluntary communication with a business — it is not intended for, and should not be used for, healthcare-related communications, symptom reporting, medical questions, or clinical inquiries. Please contact your healthcare provider directly for any medical concerns.

1. Who we are

Habit Layer LLC is a Washington state limited liability company doing business as Habit Layer. We operate the website habitlayer.com. References to "we," "us," or "our" in this policy refer to Habit Layer LLC.

2. What information we collect

Contact form submissions

If you submit the contact form on this site, we collect your name, email address, and the message you write. This information is submitted voluntarily. You are not required to provide it to use the site.

Assessment responses

If you complete the between-visit gap assessment, we collect your three multiple-choice responses. The assessment does not ask for your name, email address, or any other identifying information. However, because assessment sessions are tracked by our analytics provider (see below), it is possible that assessment responses could be associated with an analytics session identifier.

Analytics data

We use PostHog, a product analytics platform, to understand how visitors use this site. PostHog may collect information including: pages visited, actions taken on the site, browser type, device type, approximate geographic location (derived from IP address), and session identifiers. PostHog does not receive your name or email address unless you have separately submitted the contact form during the same session.

PostHog operates under its own privacy policy, available at posthog.com/privacy.

What we do not collect

We do not collect payment information, health records, insurance information, or any data through login accounts. This site has no user accounts.

3. Cookies and tracking

We do not use advertising cookies, marketing pixels, or third-party tracking beyond PostHog analytics. PostHog sets a first-party cookie to maintain session continuity and distinguish unique visitors. No data collected through this site is used for behavioral advertising or shared with advertising networks.

4. How we use information

We use the information collected through this site solely to:

  • Respond to contact form inquiries
  • Understand how the site is being used so we can improve it
  • Review anonymous assessment responses in aggregate to improve our content and offering

We do not sell, rent, or trade personal information to any third party. We do not use your information for purposes unrelated to operating this website and communicating with prospective customers.

5. Third-party services

This site uses the following third-party services that may process information you submit or that is generated during your visit:

  • Web3Forms — processes contact form and assessment submissions. Submissions are retained on Web3Forms' servers for approximately 30 days per their data retention policy. Web3Forms privacy policy: web3forms.com/privacy
  • PostHog — analytics platform. PostHog privacy policy: posthog.com/privacy
  • Cal.com — scheduling tool embedded on some pages. If you book a call, Cal.com processes your scheduling information. Cal.com privacy policy: cal.com/privacy

We do not control the data practices of these third-party services and encourage you to review their respective policies.

6. Data retention

Contact form and assessment submissions processed through Web3Forms are retained for approximately 30 days, consistent with Web3Forms' standard data retention policy. Analytics data collected through PostHog is retained per PostHog's standard terms. We do not maintain a separate long-term database of contact form submissions on our own infrastructure.

7. Data security

We take reasonable technical precautions to protect information submitted through this site. However, no method of transmission over the internet is completely secure. We cannot guarantee the absolute security of information transmitted to or from this site.

8. Children's privacy

This site is directed at healthcare practice owners and professionals. We do not knowingly collect information from anyone under the age of 18. If you believe a minor has submitted information through this site, please contact us at privacy@habitlayer.com.

9. Your choices

You may request deletion of any personal information you have submitted through the contact form by emailing privacy@habitlayer.com. Note that we may have limited ability to delete information already processed by third-party services such as Web3Forms; requests to those services should be directed to them directly.

You can opt out of PostHog analytics collection by enabling "Do Not Track" in your browser settings. PostHog respects the DNT signal.

10. Changes to this policy

We may update this privacy policy from time to time. When we do, we will update the effective date at the top of this page. We are not obligated to provide individual notice of changes. Continued use of this site following any update constitutes acceptance of the revised policy. We encourage you to review this page periodically.
